High school transition: athletics & activities registration through FinalForms

A Message from the Department of Teaching and Learning

Dear eighth grade families,

Did you know that your eighth grader can join high school athletic offerings this summer in preparation for joining high school sports? Some teams start meeting in June. This is a great way for students to meet other incoming ninth graders at their school, as well as coaches and older teammates.

How to participate:
To be cleared for any of our 2024-2025 summer and/or school year activities, go to to update your student鈥檚 information and documents.

  1. Go to聽, click on 鈥淧arent鈥 and log in.
    1. If you forgot your password, click 鈥淔orgot your password?鈥
    2. If you have never set up your聽聽account, click 鈥淣ever logged in?鈥 then enter the main email address that we have on file for you in Skyward to resend the confirmation link. Then, use the link to set up the account and login.
  2. Click on the blue box at the top of the page to register for 2024-25 year. It looks like this:
  3. Then, fill out the information or forms for each student you have in the system. Some things you may need to do are:
    1. Add sports your student is interested in participating in this summer or next year. Adding your students to a specific team ensures coaches can contact them about summer activities. Athletes and families can change their preferences later; this is not a commitment. Please only enroll your athlete in one sport per season. Students can contact the athletic office at their school if they choose to switch to another sport.
    2. Update forms and parent permissions.聽Most forms are fillable within Final Forms; you will need to upload a copy (photo, scan or PDF) of current sports physical if your student鈥檚 physical is not up to date. 聽Physicals are good for two years.
    3. Sign each form with parent e-signature.
    4. Have your student login to with their school district email聽to update student e-signature that acknowledges compliance with 香港六合彩直播开奖 Schools and state policies for athletes.

Summer sports/activities:
Many teams will have practices, camps and play in leagues over the summer. If your student is interested in a sport, make sure they are registered in and have a current sports physical on file. We encourage anyone interested in participating in sports to get a sports physical early.
